Dear Researchers,
While Preparing the Initial Application File:
All documents and forms included in the application form must be completed accurately and in full, and submitted in person to the Ethics Committee Secretariat within the Faculty of Health Sciences along with the original (wet-signed) application file.
The wet-signed application file and the version sent by email must be identical.
The wet-signed application file should be scanned and sent in PDF format to:

In addition, the relevant sections of the application file must be uploaded to the following link:

While Preparing a Revision Application:
For corrections officially requested by the committee, you must include a petition addressed to the committee, signed by the principal investigator, clearly explaining the committee’s questions item by item.
All documents that have been revised or updated (such as the revised study protocol, data collection form, etc.) should be attached to this petition.
As with the initial application, the revision file must be scanned and sent via email, and the original documents must be submitted to the Ethics Committee Secretariat for archiving.
